Sample Shipping Information

1. Place specimens and paperwork into the double box sample mailer (provided). Place inner box into ziplock bag.
   
   1. Complete the Histocompatibility Laboratory Requisition Form.
      
      
2. Transport blood specimens as soon as possible to be received by the HLA lab within 48 hours from sample collection.
   
   1. Federal Express for overnight priority delivery (use when form enclosed in kit) call 1-800-238-5355.
   2. Greater Phoenix area sample pick-up call the HLA Laboratory at (602) 343-7083 during business hours M-F; after hours call UBS Hospital Services 24 hour number at (480) 994-8016.
   
   
3. The HLA Laboratory is staffed Monday through Friday from 0730 to 1700 Mountain Standard Time. After hours, weekends and holidays are staffed on an on-call basis for urgent requests. Weekend and holiday requests must be arranged through the on-call technologist.
